Effective date: August 16, 2026
This Privacy Policy explains how Salt Lake Orthopedic Massage Therapy, operated by Bryan Lindquist, LMT ("we," "us," "our"), collects, uses, and protects information in connection with the website slmassagetherapy.com (the "Site") and our massage therapy practice.
If you use the contact form on our Contact page, we (and our form processor, Netlify) receive whatever you submit, typically your name, email, phone number, and message.
When you book a session, you are taken to MassageBook, a third-party scheduling platform. Any information you provide there (including contact details, health intake information, and payment details) is collected and stored by MassageBook under its own privacy policy, not directly by this Site. We receive your booking and the health intake information you choose to share so we can plan your session safely.
Like most websites, our hosting and content-delivery providers may log basic technical data (such as IP address, browser type, and pages visited) for security and performance purposes. This Site also uses the Meta Pixel for advertising measurement, described in the next section. This Site doesn't use Google Analytics.
This Site loads a small number of third-party resources that may set cookies or receive your IP address when the page loads:
You can control cookies through your browser settings. Blocking cookies may affect how some of these features (like the map) display.
This Site uses the Meta Pixel, a tool provided by Meta Platforms, Inc. When you visit the Site, the pixel may set cookies and collect technical information such as your IP address, browser and device type, the pages you view, and whether you reach our booking page. We use this to measure how our advertising performs and to reach people who may be interested in our services.
We don't send any health information to Meta. The pixel runs only on this Site's own pages, and we deliberately keep it off the pages about cancer care, hospice, surgery recovery, and lymphatic treatment. It isn't placed on MassageBook, where scheduling, health intake, and payment happen, and it's never used to collect or share anything about your health, the services you ask about, or the reasons you seek care.
Information collected by the pixel is shared with Meta and handled under Meta's own data policies. You can limit or turn off this kind of tracking through your Meta ad settings (in the settings inside Facebook and Instagram, or at accountscenter.facebook.com) and through your browser's cookie controls.
We use the information we collect to respond to your inquiries, schedule and prepare for your appointments, communicate with you about your care, and maintain accurate client records. We don't sell your personal information.
We share information only with the service providers that help us operate, currently MassageBook (scheduling and health intake), Netlify (contact form delivery), and Meta (advertising measurement through the Meta Pixel, limited to website activity and never your health information), and only as needed for them to provide those services to us. We may also disclose information if required by law, court order, or to protect the safety of a client or others.
Because we provide clinical, oncology-informed, and post-surgical massage, we may collect sensitive health history at intake (for example, recent surgeries, cancer treatment, or injuries). We treat this information as confidential and use it solely to plan your care safely. We don't share it outside our practice and our scheduling provider except as you direct or as required by law.
We keep client and intake records for as long as needed to provide care and to meet recordkeeping practices customary for massage therapy providers, after which they are securely deleted or archived.
You may ask us at any time to: review the information we hold about you, correct inaccurate information, delete your contact or intake information (subject to any recordkeeping we're required to retain), or opt out of promotional emails or texts (reactivation or newsletter messages will always include an unsubscribe option). To make a request, contact us using the details below.
This Site and our services aren't directed to children. We don't knowingly collect personal information from children under 13 without parental consent.
We take reasonable steps to protect the information we hold, including relying on reputable third-party platforms (MassageBook, Netlify) that maintain their own security safeguards. No method of transmission or storage is 100% secure, and we cannot guarantee absolute security.
We may update this Privacy Policy from time to time. The "Effective date" above reflects the most recent revision.
Questions about this Privacy Policy, or requests regarding your information, can be directed to Bryan Lindquist, LMT, at (801) 382-7931 or through our contact page.
